Case Management at Learning Services is the cornerstone of our service delivery model. As the primary advocate for each resident, the Case Manager builds supportive relationships between the Learning Services’ team and our residents, their families, and payers. Because each resident presents a unique and often complex web of challenges, complications, and needs, our Case Managers are experts in coordinating a wide range of therapeutic services and resources such as: Neuropsychology, Speech Pathology, Occupational Therapy, Physical Therapy, Vocational Counseling, etc.
Simply put, the Case Manager is responsible for coordinating the development of a Trans-disciplinary Treatment Plan and overseeing the implementation. Relying on collaboration between experts at each step, our outcome-oriented treatment plans are designed to facilitate the independence and quality of life of our residents.
The Case Manager collaborates with the resident, family members, sponsor, the medical community, therapists, other relevant professionals, as well as community members to negotiate, develop, communicate, monitor, evaluate and update each resident’s care and treatment plan on an ongoing basis. The goal is meeting the treatment objectives unique to each individual. To this end, the Case Manager is continuously listening, learning, planning, writing, advocating, and communicating with all interested parties.
