Case Management at Learning Services is the cornerstone of our service delivery model. As the primary advocate for each resident, the Case Manager builds supportive relationships between the Learning Services’ team and our residents, their families, and payers. Because each resident presents a unique and often complex web of challenges, complications, and needs, our Case Managers are expert in coordinating a wide range of therapeutic services and resources to support the individual Care Plan.
Simply put, the Case Manager is responsible for coordinating development of an interdisciplinary Care Plan and overseeing its implementation. Relying on collaboration between experts at each step, our outcome-oriented Care Plans are designed to enhance the independence and quality of life of our residents.
The Case Manager collaborates with the resident, family members, sponsor, the medical community, therapists, other relevant professionals, as well as community members to negotiate, develop, communicate, monitor, evaluate and update each resident’s Care Plan on an ongoing basis. The goal is meeting the program objectives unique to each individual. To this end, the Case Manager is continuously listening, learning, planning, writing, advocating, and communicating with all interested parties.
